Pain Management Directive

RECOMMENDATION

That the County of Bruce Emergency Medical Services implement the medical
directive pilot project for moderate to severe pain management.

BACKGROUND
Moderate to Severe Pain Management Directive In December 2012 the Medical Advisory Committee (MAC) for the Ontario Base Hospitals approved a pilot project allowing primary care paramedics of the Southwest Ontario Regional Base Hospital Program (SWORBHP) to administer medication for pain in the pre-hospital environment. This project is the first in the history of EMS in Ontario to allow primary care paramedics to administer medication for pain. The ability to reduce the discomfort experienced by patients is one of the most important treatments that EMS can provide. Pain control is vital especial y in the rural environment where quite often patients experience long transports to hospital. This initiative by Bruce County Emergency Medical Services is supported by the Medical Director and the Southwest Ontario Regional Base Hospital Program. All delegated medical acts completed by paramedics are delivered under the direction of a Medical Director. Medical direction is required for al medications, advanced airway support and defibril ation. The SWORBHP will monitor the new directive and its use through our existing auditing program. The moderate to severe pain management protocol will have two directives as For moderate to severe pain paramedics will be able to administer Ketorolac by injection. Ketorolac is in a class of medications cal ed non-steroidal anti-inflammatory drugs, usual y abbreviated to NSAIDs. It works by stopping the body's production of a substance that causes pain, fever, and inflammation. The most prominent members of this group of drugs are nd all of which are availablein most countries A special event pain management protocol allowing paramedics to administer acetaminophen for headache and muscle pain. The implementation of the moderate to severe pain directive involves two steps. Bruce County EMS must submit a letter to the MOHLTC Regional Field Office and request the Service’s participation in the Directive. The service must complete a training program with its staff and meet the requirements of certification as set by the Medical Director. The training will be completed local y and tentatively be completed during the first Al owing primary care paramedics to administer pain medication in the field will be one of the most important advances for EMS in Ontario. This new directive wil help many people be more comfortable than otherwise would be.
FINANCIAL/STAFFING/LEGAL/IT CONSIDERATIONS
Ketorolac costs approximately $1.30 per 30mg vial. The drug cost based on Ketorolac use for all cases is approximately $0.04 per patient carried. In 2012 Bruce County EMS carried 6,917 patients for a potential cost of $277. This estimate is based on potential drug administration. The estimated costs for the medication have been included in Medical Supplies in the 2013 budget. The cost of training staff in the new protocols and the administration of the medication is approximately $3,316 and is included in the training portion of Salaries and Wages in the 2013 budget. Saugeen Valley Children’s Safety Village

RECOMMENDATION

That consideration be given to donating one of the retired ambulances to the
Saugeen Valley Children’s Safety Village.
BACKGROUND
We have received a request (attached) from the Saugeen Valley Children’s The Children’s Safety Village is a community based project that wil provide safety programs for over 10,000 school age children from Bruce and Grey The goal of the ambulance display would be to educate, familiarize and make young children more comfortable in an emergency situation. The County of Bruce would also receive recognition on the Saugeen Val ey Since the County became responsible for the direct delivery of Land Ambulance Services on November 5, 2000 we have donated two (2) decommissioned ambulances, both to the South Grey Bruce Branch of St. John Emergency Services – Saugeen Valley Children’s Safety Vil age 02142013 THE CORPORATION OF THE COUNTY OF BRUCE

FINANCIAL/STAFFING/LEGAL/IT CONSIDERATIONS
The 2013 Emergency Services budget includes revenue of $6,000 for the sale of the two decommissioned ambulances at auction based on past experience. It is estimated that these decommissioned ambulances would bring a price of approximately $3,000 each at auction.
